Any individual having made significant contributions to the game of lacrosse, or having attained a high level of accomplishments in the game of lacrosse may be submitted by any individual for the Selection Committee to consider for Induction to the Ontario Lacrosse Hall of Fame and Museum.

The following information is required so that the person nominated can be considered for Induction. These submitions are placed in the library along with previously submitted but not yet Inducted people. Once submitted, people become eligible for nomination by the "Selection Committee" to be amongst those voted upon for admission to the Hall of Fame.
